The following information is for the 2023-24 school year.

Please note when completing online registration: ALL areas must be completed when being submitted. Any area not completed could result in your student not being able to attend school.

First time registering you will need to bring the following items with you when you have your initial meeting to set up your student's schedule:

• Proof of residency/occupancy - Residency is defined as the physical location of where the student actually lives. Please bring two (2) of the following documents:

  • Electric bill, gas bill

  • Water/sewer bill

  • Paycheck stub

  • Current renters or homeowner's insurance statement

  • Internet bill correspondence from State/Federal Programs (Foodshare, Medicaid, BadgerCare, W-2, SSI)

  • Postmarked mail from Medical Clinic.

**All utility bills and postmarked documents must be dated within the past 30 days and include both name and address.

• Proof of student's age/name - Verification of your child's name and age is required by the State of Wisconsin to enroll into a school.

  • Platteville School District requires a Birth Certificate and Student's Immunization Record.

  • Other forms of proof include:

    • Student's passport

    • Student's driver's licence or state ID

    • Foreign birth certificate, baptismal record, or an alternative document

**Wisconsin law strictly prohibits copying the Birth Certificate (BC) as it is considered a vital record.
