1. Designated personnel will be authorized in writing to administer medication to students. This authorization will be signed by the District Nurse and kept on file in the building office and nurse's office.
2. Medication to be given at school must have:
a. A written order from the physician
b. Student’s full name on the container.
c. Name of drug and dose
d. Time to be given
e. Written parent/legal guardian permission.
3. Medication will be taken by the student at a designated time supervised by authorized personnel.
4. It is the responsibility of the student, if appropriate, not school personnel, to get his/her medication at the designated time.
5. Limited quantities of the medication should be kept at school.
6. All medication administered at school will be stored in a locked drawer, cabinet, or file.
7. Parents must notify school when the drug is discontinued and the dose or time is changed. If the medication is resumed, a new order must be received.
8. The school will have established an accurate and confidential record-keeping system for each pupil receiving medication.
9. Over-the-counter medication can be administered if the pupil's parent or guardian consents in writing. Administration of these medications does not require a physicians order.
10. Newly written orders for medication from the M.D. must be renewed annually for students on long-term medication.
